Diablo IV is set in Sanctuary 50 years after Diablo III: Reaper of Souls. In the game, Lilith, the daughter of Mephisto, is summoned by cultists. She seeks to establish her power by taking advantage of the depleted forces of demons and angels.

The protagonist, a Nephalem—a race with a unique blend of angelic and demonic powers—must rise to stop her. With the world on the brink of destruction, the hero embarks on a perilous quest, gathering allies, uncovering secrets, and facing a fateful choice to save Sanctuary.

Diablo 4 follows the core gameplay formula of the series. Players progressively acquire stronger equipment by defeating increasingly challenging enemies. The game features different character classes with customisable skills and talent trees that are used to combat enemies and advance through the story and quests. Enemies are categorised into distinct monster families, each with unique traits, combat styles, and locations, allowing for synergies and specialised abilities within each family.

Five character classes have been announced for Diablo 4: Barbarian, Sorcerer, Druid, Rogue, and Necromancer, each with distinctive abilities and playstyles. The game world consists of various regions, including Scosglen, Fractured Peaks, Dry Steppes, Hawezar, Kehjistan, and Hell, with procedurally generated dungeons and an open-world setting without loading screens.
